 Frailty Social Prescribing Poster A community-led initiative in Plymouth, Age Positive helps older adults stay active, independent, and connected. Delivered by the Wolseley Trust with Improving Lives Plymouth and Timebank South West, it reduces isolation and boosts quality of life – making ageing a time to thrive.

Three interconnected elements:

  • Frailty-Focused Social Prescribing – personalised one-to-one support from a dedicated Link Worker at the Wolseley Trust.
  • Peer Support (Improving Lives Plymouth) – breaking down barriers and building meaningful social connections.
  • Skills Exchange (Timebank South West) – sharing skills and time to grow confidence, purpose, and community contribution.

Our Frailty Social Prescribing Link Worker (FSPLW) works hand-in-hand with GP practices and local partners, making sure older people get the right support at the right time by:

  • Connecting patients to community resources like Wellbeing Hubs, Memory Matters, and The Elder Tree.
  • Supporting people directly with tailored advice and encouragement.
  • Building confidence across the local workforce by sharing training and promoting frailty-aware ways of working

Dementia Advice sessions run by Memory Matters will be held at The Jan Cutting Healthy Living Centre Wellbeing Hub, launching on Thursday 30th October 2025 10.30am - 2.30pm, and on the last Thursday of every month thereafter.

Please come along to one of thier drop-in sessions to see a Dementia Advisor if you need help, advice or support with anything memory related.

Free Health & Wellbeing Coaching for PL2 Residents – with our accredited Coach LottaLSS HC Poster

Are you ready to make positive, lasting changes in your life?  Lotta, an accredited Health and Wellbeing Coach with NHS experience, is offering up to six free one-on-one coaching sessions exclusively for residents in the PL2 area.

Based at the Jan Cutting Healthy Living Centre, Lotta provides a personalised, supportive approach to help you improve your physical and mental wellbeing. Through confidential, goal-focused conversations, you’ll explore what matters most to you, set achievable health goals, and build confidence to overcome challenges. Whether you’re living with a long-term condition, facing lifestyle hurdles, or simply looking to feel better in yourself, health and wellbeing coaching offers anon-medical, empowering path forward.  

This service is ideal for individuals who are ready to take an active role in their wellbeing and make meaningful progress, though it’s not suitable for those inimmediate clinical or mental health crisis.

In close proximity to the Wolseley Hub and in the grounds of the Wolseley Business Park, the Trust has

its own community garden. The aim of the garden is to bring together a range of local partners to deliver:


• Educational outdoor learning space for our Youth Groups, Community nursery and local schools
• A space for local people to learn new skills.
• Timebank and volunteering opportunities

Community Roots Group

 

     Get involved!

     In early 2024, we created ‘Community Roots’, a vibrant gardening group. Our dedicated team of local

     volunteers meets weekly at the Patchwork, where they’ve been cultivating a thriving community garden. 

     You don’t have to be an experienced gardener (but of course you’d be very welcome!)

     This group is for people looking for friendships and connection whilst being outdoors enjoying nature and

     making a difference to a lovely community space - our Wolseley Trust Patchwork Garden.

     

 

 

We dig, we plant, we grow, we share, we tidy and most importantly, we do it TOGETHER with the help of the Green Communities Team

Wolseley Trust Compassionate Café

A Compassionate Cafe is a friendly, welcoming place to have a cup of tea or coffee with a Compassionate Friend who is there for company, to lend a friendly ear and provide a place to stay connected to others.

Visit our Compassionate Cafe at Wolseley Community Hub (next to Coop) every Tuesday at 11-1pm.

We welcome people from the local area and taking this new step towards supporting each other through difficult times. 

The group has become a friendship group outside of these sessions, with the attendees regularly enjoying meals out together as well as daytrips – most recently to Clark’s Village and a holiday to Eastbourne last year.

Keyham Counselling Service

Established in the wake of the Keyham tragedy the Wolseley Community Hub is proud to offer a trauma Counselling Service to those impacted by the Keyham tragedy. Co-funded with Plymouth City Council, the service offers individual therapy sessions, delivered by our trauma trained counsellor Dinah who is committed to providing compassionate and supportive care. The service has continued to received referrals through 2023-2024.

Plymouth Cancer Champions is a 3-year project funded by Macmillan Cancer Support in collaboration with The Wolseley Trust, Age UK and Zebra Collective.

At the heart of the Plymouth Cancer Champions Project is a mission to raise cancer awareness, provide compassionate support, and empower our community through connection and education. The team, trained in trauma-informed care and solution-focused approaches, is working closely with local organizations, faith groups, and individuals to create a network of Cancer Champions—dedicated volunteers who act as “Community Connectors.”

Through initiatives like our Cake and Cuppa Sessions, community events, and tailored social prescribing, we’re breaking down barriers to support and offering people a safe space to talk, share, and learn. Our Social Prescriber has already made meaningful connections, with life-changing feedback such as: “You’ve given me permission to recover.”

With vibrant partnerships, outreach efforts, and innovative programs like cancer cafés, we’re fostering inclusivity, kindness, and hope. Together, we’re building a stronger, more resilient Plymouth community, where no one faces cancer alone.

The Community and Wellbeing Programme

Based in Stonehouse, the Community Health and Wellbeing project is a collaboration with Waterside Health Network that takes a grassroots approach to supporting families with their health and wellbeing within their homes and local area. The programme’s unique concept, ensures that the CHWW’s (Community Health and Wellbeing Workers) visit residents within their own home at least once a month, acting as a link to Primary Care, ensuring the family unit are supported with any social, mental or physical health needs.

Building on a model that is now embedded into Brazil’s Family Health Strategy, the CHWW programme is a systematic, universal, comprehensive and integrated approach that is designed to build strong relationships with the local community to enable health needs are proactively met. With the CHWW’s being integrated into primary care, they can link effectively with relevant services to help promote vaccinations, screenings and management of chronic diseases. Alongside this, as local members of the community they support, the CHWW’s are able to encourage social engagements and link residents with voluntary and community sector organisations.

Our team of Saryah, Miranda and Matt are passionate advocates for their local community, embodying a person-centred, ‘what matters to you?’ approach and are already seeing the positive ripple effect of the programme.

At the Wolseley Trust Tenancy terms have been designed to be flexible and uncomplicated, with an “easy in easy out” tenancy agreement. We offer free on site business advice and support available from Wolseley Community Development Trust staff and other agencies.

The details are summarised here:-

  • Two months rent as deposit will be required. The sum will be refunded at the time of vacation of the premises subject to a satisfactory inspection of the Unit.
  • Rent is due on the first day of each calendar month payable in advance by standing order or BACS Payment. 
  • A tenants contribution of £120.00 + VAT per annum is payable towards general landlords servicing costs.
  • Tenants will be liable for the costs relating to the provision of normal services to their units including: telephone, electricity, gas, water and sewerage charges; trade waste and Business Rates.
  • Tenants will be responsible for the repair and maintenance of internal aspects of their unit and will be expected to maintain the unit to a standard acceptable to WCEDT.
  • Internal modifications to units may be made at the tenant's cost and subject to prior written agreement with WCEDT.
  • Upon vacation of the premises, the tenant will be responsible for all costs associated with reinstating the unit to it's original specification.
  • One month's notice in writing by the tenant will be required on vacating a unit.  
  • Please Note: retail trade is not permitted on the Business Park.
  • Full details of costs and copies of the tenancy agreement and tenancy handbook are available on request.

At Wolseley Trust Healthy Futures, we've developed our Social Prescribing and Personalised Care service building on the successful Healthy Living Centre model and a long-standing wellbeing partnership with Plymouth City Council. Our main goal is to ease the pressure on traditional care systems by offering a holistic package of local support. We connect people with valuable community services to boost their mental and physical wellbeing, tackle social isolation, empower them to take charge of their health, and strengthen both individual and community resilience.

Our Impact and Approach:

Since 2017, our service has been directly subcontracted through the NHS Long Term Plan, specifically tasked with delivering the personalised care requirements of the Network Contract Directed Enhanced Service (DES).

During 2023-2024, we successfully operated across 6 Primary Care Networks, encompassing 33 GP surgeries. Our model aligns with the NHS Five Year Forward View, focusing on supporting people more holistically. We achieve this by linking health with social care and other support needs, always emphasising prevention and early intervention to help people thrive.

The Activ8 brand is a Wolseley Trust physical activity programme and Gym based on Wolseley Business Park. In partnership with Ford Youth and Community Gym (a local registered Charity), the Trust directly commissions a gym and a range of physical activity interventions.

Ford Gym is the community’s gym, run by members for the members, and offers a unique social, almost family like training environment for residents of all ages and abilities. Terms and Conditions of Hire for both the Conference Room and Community Building

The Cancellation Policy for our facilities hire is as follows;

  • 7-14 days notice - No Charge
  • 4-7 days notice - 50% of room charge
  • 2-3 days notice - Full Cost

Tea and Coffee is normally included in the hire price for the conference room and we can arrange this alos, at extra charge, if you hire our community building. Hirers are requested to wash and dry all cups and saucers used, and to leave the room clean and tidy.  The Trust can also provide free use of the following equipment:

Flipchart, TV and DVD, Porjector and Screen if requested and booked in advance.  

Please note:  Our conference room is set up as boardroom style.  If you require any other tpyr of set up, i.e. theatre style, please let the Wolseley Trust know in advance of your booking.

Our payment conditions are as follows;

  • Conference Room and Community Building Hire requires payment within 14 days of the invoice date we send at the end of each calendar month.

Scott Business Park stands as a prime example of successful urban regeneration, transforming a former hospital site into a vibrant commercial hub.

This ambitious project was undertaken in partnership with Plymouth City Council with funding from the Single Regeneration Budget, Southwest Regional Development Agency, and Government Office for the South West.

In partnership with the Wolseley Trust, Pollenize launched a biodiversity monitoring project for 2023-24 aimed at enhancing the site's native wildflower patches and continued monitoring of plant and insect species on site.  Green spaces in business parks offer more than aesthetic value—they play a crucial role in supporting biodiversity, improving air quality, and creating a pleasant environment for employees and visitors. Projects like Floradex demonstrate how businesses can actively contribute to local conservation efforts while reaping the benefits of a healthier and more vibrant environment.

  • Two month's rent as a deposit will be required. This sum will be refunded at the time of vacation of the premises, subject to a satisfactory inspection of the unit
  • Rent is due on the first day of each calendar month payable in advanced by standing order or BACS payment.
  • Monthly service charges apply at Scott Business Park Rental. These charges are based on tenants contributions to common services.  This covers the following:
    • Grounds maintenance
    • Business park lighting external and communal areas
    • Security services, CCTV cameras, etc
    • Service and maintenance contracts.  
  • Tenants will be liable for the costs relating to the provision of normal services to their units including: telephone, electricity, gas, water and Local Authority business rates.
  • Tenants will be responsible for the repair and maintenance of internal aspects of their unit and will be expected to maintain the unit to a standard acceptable to WCEDT.
  • Internal modifications to units may be made at the tenant's cost and subject to prior written agreement with WCEDT.
  • Upon vacation of the premises, the tenant will be responsible for all costs associated with reinstating the unit to it's original specification.
  • One month's notice in writing by the tenant will be required on vacating a unit.  
  • Full details of costs, and copies of the tenancy agreement and tenancy handbook are available on request.
